Membership Application Kit
The complete kit, with necessary information and forms to become a Screenrights member and register your title(s). Alternatively you can download the individual forms separately:

• About Screenrights Registration Forms (Form A)
  Information about the Collection Services that Screenrights administers. A copy is also enclosed in Membership Application Kit.

• Membership Application Form and Agreement (Form B)
  This form is used to apply for membership and becomes the basis of your agreement with Screenrights. A copy is also enclosed in Membership Application Kit.

• Warranty (Form C)
  A general warranty, or legally binding promise, that indicates how you will be claiming royalties for the majority of the titles you register with Screenrights. You only need to complete this form once.A copy is enclosed in Membership Application Kit. Also see sample form: warranty (76k)

• Title Registration (Form D)
  Used to register one-off programs such as feature films, documentaries and short films, and to register the main title of a serial work. A copy is enclosed in Membership Application Kit. See also sample forms: documentaries (90k); features (91k).

• Season & Episode Details (Form E)
  Used to provide production details of the seasons and episodes associated with a serial work and should be completed for each season. A copy is enclosed in Membership Application Kit. • Bank Details
  Screenrights pays royalties directly into a nominated bank account.  Use this form to provide your account details.
